ICICI Securities Maintains Star Health Buy Rating, Sets Rs 605 Target

ICICI Securities has kept its ‘Buy’ rating on Star Health and Allied Insurance with a revised target price of Rs 605, down from Rs 690. The brokerage highlighted strong retail growth and a return to underwriting profitability as key drivers. The stock is currently trading at approximately Rs 511, as investors remain cautious regarding potential regulatory changes in the insurance distribution sector.

ICICI Securities has maintained its ‘Buy’ rating on Star Health and Allied Insurance, while revising its target price to Rs 605 per share from the earlier Rs 690. The stock is currently trading at approximately Rs 511.30. This adjustment in the target price reflects a cautious outlook on the wider insurance industry, specifically concerning evolving regulatory norms for distribution, rather than a critique of the company's internal operations.

Financial Turnaround and Growth

The company has demonstrated a notable recovery in its financial performance for the first half of the 2027 fiscal year. Star Health reported a net profit of Rs 814.6 crore, a significant jump from the Rs 517.8 crore recorded in the same period the previous year. A major driver of this improvement is the return to underwriting profitability—which refers to the profit earned purely from insurance operations after paying claims and expenses. The company posted an underwriting profit of Rs 210 crore in the first half of fiscal year 2027, reversing a loss of Rs 24 crore seen in the same period last year.

The improvement in performance is also visible in the combined ratio, which measures the claims and expenses paid against the premiums earned. The ratio improved to 97.9% in the first half of the current fiscal year, compared to 100.3% in the year-ago period. A lower combined ratio is generally viewed as a positive sign of operational efficiency.

Operational Strategy

Star Health has shifted its focus toward the retail segment, which currently accounts for a dominant 33% market share in the industry. To stabilize margins and manage the impact of medical inflation, the firm has initiated price adjustments across nearly 80% of its product portfolio. Additionally, the company has actively reduced its exposure to group insurance products, which historically carry higher volatility. This, combined with its proprietary agency network, remains a central part of the company's strategy to navigate a competitive market.

Sector Challenges and Investor Risks

Despite the operational improvements, the insurance sector is currently navigating a period of regulatory uncertainty. Investors are closely watching for potential changes regarding insurance distribution reforms and commission structures, which could impact future profitability across the industry. Furthermore, the company faces ongoing pressure from intensified competition and medical inflation, which can drive up claim costs.

Another point of interest for shareholders is the historical friction between health insurers and hospital networks regarding cashless service tariffs and claim settlement processes. While the company has taken steps to manage these costs, any unexpected spike in claims or further regulatory directives—such as those regarding portability norms and claims settlement compliance—could create short-term volatility. The next important monitorable for investors will be the company’s ability to maintain its margin expansion and market share while complying with the evolving regulatory landscape.
